Referrals / Authorizations
It is the policy of this office to require a current referral from patients whose insurance requires them to obtain a referral prior to treatment by a specialist. As you can imagine, seeking medical services in todays healthcare environment without the proper paperwork creates unnecessary delays, expense and stress for all involved. Based on the requirements of your health insurance plan, you are responsible for having a referral in hand.
We understand that confusion, events and circumstances sometimes cause a patient to arrive without the required referral. In order to keep our office running efficiently for all our patients, we ask you to reschedule at a time when the referral is available. We do offer the option of being seen as a private pay patient. This will allow you to keep your appointment with no delay. This is our only option in allowing you to be seen without the necessary referral. We do not have the resources to address concerns with your insurance company, nor attempt to make a referral retroactive in order to pre-date the referral. Insurance companies are typically not cooperative in this attempt. Trying to communicate with your primary care physician once at our office, and anticipating a faxed referral prior to your appointment time here has not proven successful in the past and we are unable to offer that option.
For these reasons we ask you to understand how we can help you as our patient by asking you to either reschedule for a future appointment or make the necessary payment for todays visit.
Payments & Financials
We are committed to providing you with the highest quality medical care, in an efficient, timely and cost-effective manner. We hope that by providing you with our policies in advance we can prevent any misunderstanding or frustration at the time of your visit.
If you incur a balance with our office, we may ask for a valid Credit Card (Visa, Mastercard or American Express) to guarantee your appointment. If you cannot supply a valid Credit Card, we will not be able to schedule your appointment. Our billing service will send statements on a monthly basis for amounts that your insurance provider deems to be 'Patient Responsibility' on your Explanation of Benefits. If you do not pay the balance on your account, you will receive notification that your account will be turned over to a collection agency. Once your account is placed in collections, your credit card may be charged the full amount that you owe.
By providing us with a credit card, whether in person, on the telephone, or by other means, you agree that we may charge the amount noted on your most recent statement. Statement Payments can be made securely online at: https://www.lsinj.com/payments
When making an appointment, it is your responsibility to confirm with your insurance company that the physician is currently under contract with your plan. If your plan requires that you have a referral prior to seeing a specialist, please contact your primary care physician so that you will have the referral in hand at the time of your appointment. We do not accept faxed referrals. If you do not bring your referral with you to your appointment, we will need to reschedule your visit, unless you choose to be seen without using your insurance benefits and pay for your visit in full.
We will gladly file your insurance claim on your behalf. We allow 45 days from the date a claim is filed for the insurance company to pay. If your insurance carrier DOES NOT pay within this time, you will be responsible for the entire balance. We will not become involved in disputes between you and your insurance company regarding coverage and/or policy benefit criteria, i.e. deductibles, non-covered services, co-insurance, coordination of benefits, pre-existing conditions or reasonable and customary charges, etc. other than to supply factual information when necessary. You are responsible for the timely payment of your account.
Please arrive for your appointment at least 10 minutes prior to your appointment time, so that any paperwork that needs to be completed, can be done before you are scheduled to see your provider. Please also bring your current insurance card with you to EACH VISIT. Without the insurance card, we will be unable to file your insurance, and you will be responsible for the charges for the day. On follow-up visits, you will be asked to verify demographic and insurance information so that our records remain up-to-date.
Please be prepared to pay for the current visit as well as any past balances on your account. Payment of co pays, deductibles or fees for non-covered services will be required at the time of service. Estimated patient responsibility for surgical procedures will be determined by insurance benefit coverage and collected at the time of service. For your convenience, we accept cash, checks, Visa, Mastercard or American Express & eWallet applications such as Apple Pay.
We do our best to keep to the schedule. When a patient arrives late, it is impossible to stay on schedule. If you arrive more than 10 minutes past your scheduled appointment time, you will be rescheduled so that other patients are not inconvenienced.
If you must cancel a scheduled Medical Dermatology appointment, we ask that you call by 10:00 am one (1) business day in advance.
If you must cancel a scheduled Laser or Cosmetic Dermatology appointment, we ask that you call by 10:00 am two (2) business days in advance.
Appointments are in high demand, and early cancellation will give another patient the opportunity to access timely care as well. To cancel a scheduled appointment, please call 973-635-5050 If you do not speak to a receptionist, please leave a detailed message with our answering service. Late cancellations will be considered a 'no show'.
A 'no-show' is someone who misses an appointment without cancelling it by 10:00am one (1) business day in advance.
No-Show/Late Cancellation Fees: We require a valid Credit Card (Visa, Mastercard or American Express) to guarantee your appointment. If you cannot supply a valid Credit Card, we will not be able to schedule your appointment.
Any Skincare / Spa appointment that is broken with late notice will result in a fee of 50% of the original service charge.
Any Cosmetic procedure appointment that is broken with late notice will result in a fee of $100.00
An Insurance Waiver may be required to acknowledge understanding of your responsibility for paying for non-covered services. In dermatology, there are many procedures that are considered by Medicare and private insurers as non-covered, including elective removal of skin tags and seborrheic keratosis, cosmetic treatment of facial spider veins, removal of whiteheads, as well as others. If you are coming in for a non-covered service, please be prepared to pay for the service in full. Cosmetic procedures including, but not limited to chemical peels, sclerotherapy, photo rejuvenation, laser, fillers and Botox treatments are not covered by insurance and claims will not be filed for them.
The parent(s) or guardian(s) accompanying a minor are responsible for providing current insurance information for the minor and/or payment in full for services provided. Unaccompanied minors must have a written authorization for medical treatment signed by the parent or guardian before treatment can be rendered.
